Use this form to log your controlled substance waste (expired medications, dropped tablets, broken tablets, etc). It is designed to print front and back to give you more room for logging and comes with one form for C2’s and one form for C3-5’s. There is also a place to sign them in the waste and a place to sign them out after destruction or transfer to a reverse distributor.

Use this form to efficiently document all the information needed to transfer prescriptions. Each copy will hold up to four prescriptions. It has space for everything you might need! Patient info, insurance info, all pharmacy info (including DEA For controls), and prescriber info.

Prescriber info is separated for each prescription – there’s nothing worse than writing all the drug info for a second transfer down only to find out the doctor is different and then having to try to make the second doctor’s info fit.
